Module 7: Transfer, Leave & Discharge and Miscellenous

A patient is discharged to a nursing home from an approved centre, is the approved centre service obliged to provide a follow up service to the pt in nursing home? If so in what capacity and for how long a period? Is there legislation or guidelines to provide information re this?

A patient’s care and treatment plan should include a discharge plan, which includes after care. Therefore it is a clinical decision to determine a patients’ needs post discharge.

The finalisation of the consultation process of the Mental Health Commissions draft Code of Practice on Admission, Transfer and Discharge is currently ongoing.
